Could you be reincarnated or have 2 consciousnesses at once?

I am wondering whether it would be possible to reincarnate someone. For example if you died and someone created another person with your exact thoughts and memories, would that be YOUR OWN consciousness? And if a Boltzmann Brain were formed somewhere in the universe with your exact memories while you were living, would that mean you have 2 consciousnesses?
